- Improvements recommended but not blocking
When to FAIL:
- Failing tests
- High-severity security issues
- Major standards violations
- Insufficient AC coverage
- Breaking changes without migration
When to WAIVE:
- Known issues accepted by product owner
- Technical debt acknowledged for future fix
- MVP constraints require compromise
- Always document waiver rationale
Blocking Conditions
HALT and report if:

Advisory, Not Blocking
Remember: QA provides advisory guidance. The goal is to:

Use CONCERNS for issues that should be addressed but don't prevent deployment. Use FAIL only for serious issues that genuinely should block deployment.
Error Handling
If testing skill fails:

- Provide manual testing guidance
- Gate decision: FAIL (tests not executed)
If standards/architecture missing:
- Note missing documentation
- Perform best-effort review
- Recommend running standards/architecture skills
- Gate decision: CONCERNS (cannot validate compliance)
If story incomplete:
- Report incomplete implementation

- Cannot perform QA review
- Return error, suggest completing story first
